What are cataracts?

Put simply, a cataract is a hardening and clouding of the lens of the eye. Often it is referred to as any opacity (i.e. something that is difficult to see through) within the lens. Cataracts can cause serious visual problems, including blindness, if left untreated. This is because the lens of the eye that is used in focusing. In a normal eye the lens is clear and the image seen is sharp and defined. However, when a cataract leads to the lens becoming cloudy, the image will be blurred - a bit like looking through frosted glass all the time.

In its early stages, a cataract may be very small (an incipient cataract) and not interfere with vision at all. As more of the lens is affected (an immature cataract) vision becomes increasingly blurred. Eventually, the entire lens can become cloudy (a mature cataract) and all functional vision lost.

Do I have cataracts?

Many people, old and young, can develop a hardening of the lens of the eye. This makes the lens less translucent and the lens becomes grey or pearly in appearance. Although many people think that this process is the development of cataracts, it is in fact nuclear sclerosis. Despite the appearance of an eye affected by nuclear sclerosis, nuclear sclerosis does not usually significantly interfere with a persons vision.

Your optometrist will be able to diagnose nuclear sclerosis as opposed to true cataracts.

Cataracts can develop rapidly over a matter of only a few weeks or they may develop very slowly over a matter of years. They may also affect one or both eyes.

It's important to remember that cataracts are not:-

1. Cancerous
2. Do not affect other structures in the eye apart from the lens
3. Infectious - they cannot be spread form one eye to the other.
4. A cause of irreversible blindness.

What causes cataracts?

1 Most cataracts are inherited.
2 Cataracts may appear spontaneously with old age. This does not usually happen until a person is at least 60-70 years old.
3 Diabetes mellitus sufferers are also more susceptible to developing cataracts.
4 Poor nutrition when young may also result in cataracts developing.
5 Trauma to the eye may cause cataracts.
6 Cataracts can sometimes also appear secondary to other eye diseases.

How are cataracts treated?

Once your optometrist has diagnosed that you have cataracts, you will probably only be offered one course of treatment and that is surgery. You may be referred to an optometrist where you will be assessed in order to establish if you are a suitable candidate for surgery.
If it is decided that you are suitable candidate for surgery you will be offered a procedure called phacoemulsification. This is the same procedure that is performed on humans and uses the same type of equipment.

A small incision is made in to your eye(s) and a hole is then made in the capsular bag that holds the lens. An ultrasound probe is inserted into this hole and the cataract is ultrasonically broken down and removed. The entire lens has to be taken out and is replaced with an artificial lens known as an IOL or intraocular lens. Tiny sutures are used to close the eye. If both of your eyes are affected by cataracts, both eyes are usually operated on at the same time.
